For grounds under a separation agreement, the husband and wife must sign written agreement, acknowledged by a notary public, filed with the county clerk, and a year must pass.

There are no grounds. Two people can decide to separate for any reason.

In order to convert the separation to a divorce, you can both sign a separation agreement, file it, and wait one year.

Alternatively, you can pursue a divorce on the grounds of abandonment after one year from the date one spouse moves out.
