Legal Question in Family Law in New York

My son's father has been recently diagnosed with severe depression and has a history of a suicide attempt. He has been attending counseling sessions. He hasn't started any medication yet because he thinks he doesn't need it. I'm nervous about letting him visit our 8 month old son. He has threatened to take him out of state. He lives in another state but is returning to live in our state...so visitation was never an issue before. We have not been to court. Can I request supervised visits?
