I do not know about a statute of limitations. However, if the QDRO affects your ability to obtain a pension, and the QDRO is not filed, it will adversely affect you. Your ex would be able to transfer or dissipate money from his pension.

You should be giving your ex written notice of his failure to prepare and file the QDRO and explain that if you have to do it, you will pass the cost to him. Then, if he still fails to prepare and file the QDRO, you should prepare and file same, and then sue your ex for the cost.
