taking maiden name back after divorce

what do I have to do to take my maiden name back. I am divorced.

You should contact the clerk of the supreme court in your area, you may be able to bring a simple action on your own and get your name changed back. I haven't done it in NY, but it is a simple process in TX.

If the divorce decree says you can resume your maiden name then that's all that is necessary.
