termination of parental rights

Hi! I'm a new attorney and I'm trying to help a friend who has very little money. I am hoping that a more experience attorney will guide me. She wants to have her ex-husband's parental right terminated because he never paid child support. He does call his two children every six months. He is in jail in Georgia for grand larceny. There is a great possibility that he will consent to his termination of parental rights. Does the ex-husband have to be served? Is there a special form needed for termination of parental rights? If there is, where may I find it. Thanks.

Absent a Court Ordered Adoption, by another, parental rights and obligations cannot be terminated.
