Legal Question in Family Law in New York

uncontested divorce

How do I know whether to file form UD-1 Summons with Notice or form UD-1a summons with a verified complaint? I have a legal separation agreement that was filed in the county clerk's office already.

You should attach Blumberg form No. T158 "summons in an action for divorce".

The form is very simple and requires very little trouble to fill out. The clerks want to see the form and will make your life easier if you give it to them.

You may not be required to submit it, based on the separation agreement already covering the issues, but make your own life easier.
