Legal Question in Civil Litigation in New York

Information Subpoena

I'm trying to fill in the blanks for the question section of an information subpoena. Unfortunately it's not very clear what is supposed to go where. It says: _________ being duly sowrn deposes and says: that deponent is the _________ of ________ recipient of an info. subpoena herein and of the original and a copy of the questions accompanying said subpoena. Does anyone know what should go in the blanks? It's for Suffolk County, NY.

It's asking you for specific information under the penalities of perjury, as for the specifics, I couldn't tell you without knowing more about the speicifics of the subject matter of the subpoena, if you'd like help you can contact my office for a consultation.

The first blank is for the name of the person filling out/swearing to the form. If that's you, your name there.

The second and third blanks are if you are filling out the form on someone else's behalf. In other words, how do you know the info.

Example: "John Doe" being duly sworn......that deponent (that is John Doe) is the "bank manager" of " XYZ Bank" recipient of....

Example 2: that deponent is the "landlord" of "John Doe"...
