Legal Question in Civil Litigation in New York

information subpoena

I'm trying to prepare an information subpoena, and I'm stuck. I've prepared subpoena duces tecum and subpoena ad testificadum, but in both those instances the laguage was to appear in court... Would anyone know the exact language for an information subpoena that has to be so ordered (NYC) and therefore needs to be perfect? Also, do I put in a return date? thank you in advance.
