Legal Question in Civil Litigation in New York

reporting person for contempt

What is my next step when person I sent Information Subpeona to, ignored it? I would like to report him to the courts but I don't know how to do this. I am told I have to prepare a legal paper to present to the Judge. I have no idea how to this and where to present this. This is a civil court affair, I live in New York city. I won a judgement against defendant for ruining my carpet they cleaned. They never paid me any monies they owe me.
