surveillance: How to get?
Hello,
RE: NYC small claims court.
How do I obtain in store video/audio surveillance?
For example, do I need to fill out a form and Fedex it to the store?
What form do I need to fill out?

I am assuming that you mean to obtain a copy of a store's surveillance tape. You would have to prepare a subpoena for the information and go to the small claim's clerk to have the subpoena signed. You would then have to hire a process server to serve the subpoena on the store.
