Does my mother need a medical power of attorney if my son & I are her only relatives & would name us anyway?
If it is needed, can my son's wife & family members sign as witnesses?

The best advice is to get a medical power of attorney or living will and yes, your son's wife and another relative can be a witness to her signature.
