Canadian in America on K-1 Visa
I am in America now, and have
entered on a K-1 visa. My fiance and
I just got married and I am now
applying for the Green card. I have
the application forms, but read that I
do not have to have the medical
exam done again as I had it done in
Canada already. I read mention of a
vaccination supplimentary form, and
cannot find the form anywhere on
the USCIS website. Does this form
exist, or do I simply photocopy my
vaccination records to send with my
application?

Madam,
The supplement should have been included with the original medical exam. The form is completed by a USCIS approved physician for submission to the USCIS.
