Legal Question in Immigration Law in New York

i'm a us citizen in the process of preparing the k-1 fiance visa application for my fiance in the Dominican Republic. He has a court order in DR to pay child support for a child that is not declared as his on the child's birth certificate. Do I need to enter the child's name on the application as one of his children.
