Filing INS forms applying for Green Card through marriage
I'm applying for Green Card through marriage. When I went for the medical exam, the doctor told me I needed immunization records, which I don't have, and a separate INS form (besides I-693) for immunizations. What form is this?

INS designated surgeons should have the form. If the doctor doesn't have the vaccination form, go to another doctor. If you don't have vaccination records, then you must get all age appropriate vaccinations.
