I am going to hire models for my website. What type of form do I have them sign prior to displaying their pictures on my website?

You need a release prepared that you can have them signed to grant you all the necessary rights to display their likeness on a website or in another medium and for whatever other purposes you intend. Models should be paid for their modeling services. If you deal with minors, their parents would need to sign.

You don't describe what your website will do. For example, if you are establishing an agency there are legal requirements you need to meet. If these are models who will simply be on your website to dress up the site, you need to ensure that you acquire all rights to use the model's likeness. If a photographer other than you is shooting the models you need to make sure you acquire the copyright in the photos.
