Legal Question in Intellectual Property in New York

I am writing a book that includes using other peoples personal experiences. I believe I need a release form to get permission to use their story. Do I have to compensate them for using their stories as well? If not, Where can I find a release form saying this?

Yes, you do need a release form.

You do not have to compensate them for using their stories.
