Legal Question in Medical Malpractice in New York

Medical Conduct

My daughter's former Pediatrician lost her medical records and did not report her vaccine to the Citywide Immunization Registry which she is mandated to do and now I have no record of my 20 month olds vaccines. I already reported her to the city and the state but to no avail. What can I do now?

Perhaps you could have the former pediatrician, or his/her office staff, prepare an affidavit attesting to the giving of the vaccines to your children, and perhaps the Immunization Registry would find that acceptable. It would at least be helpful to have for future providers of care to your kids. Good luck.
