You will need to execute a deed from yourself to you and your domestic partner. Depending how you want to hold title, you need to specify a joint tenancy or joint tenants with right of survivorship. There are also some other forms that need to be completed and filed. You can probably do it yourself, but for a small fee you can hire a lawyer and avoid the headaches.

Real simple. Go to any legal stationary store and ask for a quit claim deed form. The present grantee will then fill out the form conveying the property to himself and partner jointly and sign it. Have his signature acknowledge and file it with county clerks office.
