home ownership

My home is in my grandmother's

name.There is a possibility that she could end up in a nursing home and I don't want my home taken away. How do I

get my name put on the title

instead of hers ?

Assuming that you are certain that there are no other third parties claiming to have title to your home, then have your grandmother sign a BARGAIN AND SALE DEED trnsferring her title and claim to the home to you.
