You should have your attorney prepare a new deed transferring the property for $1.00 from you under your former name to you under your new name and file that new deed with your county clerk's office. It'll cost you some fees both to the attorney and to the county clerk, but the transfer will be well worth it when you go to sell the property.

Depending on the terms of any mortgage that you have on the property, you may need to get the written permission of the mortgage holder before you change the deed. That should be easily done in just a phone call and letter to them explaining what the change is about.
