Petition for declaration of death EPTL 2-1.7

I'm paralegal student on a mission in finding model forms on how to write a Petition for declarationof death persuant to EPTL2-1.7. I don't know how to do so and just want a Model form inorderto create one of my own or the exact form and forme to fill in the blanks. Please help.
