Quiet Title
are there forms that can be used to sue for quiet title and if so where can I get some ?

There is no form. You file a summons and complaint.
I suggest you need an attorney for this type of action.

An action to quiet title in commenced under NY Real Property Law.
Debra's answer is not entirely correct. Forms may be found at your local law library - which is usually located in the Supreme Court. However, unless you know what you are doing it will be difficult to properly commence and complete the action.
