What is standard contract for NYC Co-op owner to rent his apartment?

I own a NYY co-op apartment and will be renting (subletting)it to a rentor. The rentor has been approved to sublet by the bldg co-op board.

What standardized NY State legal contract should we sign as a legal agreement between me, the owner, and my rentor?

You can obtain a standard blumberg form from your local stationary store. However, I would suggest going to an attorney since there are some important clauses that may not be available in the blumberg form.
