Legal Question in Wills and Trusts in New York

I have 5 adult children. I live in a mobile home which I own, and have very little money to leave my children. Living in NY, I want the basic will, power of attorney If I become disabled, and for after I die. I am divorced from a second marriage. What are the basic forms I need? My finances are very, very simple.

1. A simple will properly executed conveying your assets to your beneficiary.

2. Living will and health care proxy for someone to make medical decisions for you, should you be unable to do so.

3. Power of Attorney for someone to make decisions for you should you be disabled.

This can be done using forms available at any stationery store, but some forms require language that may not be in the off-the-shelf forms. To be safe, even if legal fees may cost more, use an attorney. This is a response to an Internet question and the response is not intended to be legal advice or as creating an attorney-client relationship.
