Legal Question in Wills and Trusts in New York

My cousin mother had a stroke, he has been living there taking care of her before the stoke occured. She is now in a nursing home and the insurance is running out Medicaid is interested in her assets 1) is he now a legal carekeeper. He is also the benefactor of her property 2) Can he sell or take ownership of the house to protect his interest, does he have a legal right to protect him self from medicaid
