Durable Power of Attorney

I am interested in doing a durable poser of attorney for health reasons. I would like to know if I have to file with the state of New York, if so.Where does it have to be filed or mailed in. I appreciate your time and help.

Re: Durable Power of Attorney

A durable power of attorney for health or "health care proxy" does not need to be filed with the State. You should, however, give copies to your attending physicians, local hospital, family members and your agent appointed in the proxy.
