Legal Question in Wills and Trusts in New York

power if attorney

Is there one form for Durable Power of Attorney, that is valid in the state of New York and Florida. Or do I need two seperate forms to cover me.

he docum,ents are the same, just the notary aacknowledgement clauses differ. Some states are particular about this language and if the correct language is not used, it may prevent the document from being recognized as valid.
