Power of Attorney

I am not married to this man that I have lived with for 28 years. Now he wants to give me power of attorney. I have that form do I need any other forms to be signed by him to accompany the Power of Attorney

Depends on the nature and purpose of the POA. A POA will not allow you to make healthcare decisions, and depending on its nature may or may not be effective if the grantor gets sick or otherwise becomes incapacitated to a degree that he no longer becomes able to grant consent.
