waiver of process consent to probate

i have a question,on the form there is a place for my signature etc.where the relationship is do i write self or do i write my relationship to the deceased?

If you received the form from an attorney I would leave it blank.

However, if you do want to write in the blank, it would be your relationship to the deceased.
