Put the items down on the form in the spaces provided. If you still owe something for the items include the "amount you owe" and the name of the store, bank, etc. on the space for "to whom money is owed". Don't forget to list your bank account on the form but do not put the account number on the form for security purposes, just the name of the bank and how much you claim.
