You need to fill it out, serve it on the other party or their attorney and file it with the Court within 20 days from when you recieved them. I recommend you contact your local Legal Aid office or an attorney for assistance. These forms protect your property from being taken to satisfy the judgment.

I can help you fill out the form properly. I charge $50 to review the form with you. Please contact me at [email protected] if interested.

If you ignore this, then you will waive any exemptions that you have. By waive, I mean that if you do not fill out the form, then the sheriff may be able to seize and sell any assets that you own free and clear and levy on your bank account if you do not take steps to protect it. Of course, this depends on the types of assets that you own, how they are titled and your sources of income.
