Forgery or Fraud?
My son and his wife are separated and before she left she took out a $12,000 loan and signed his name and used his social security number to get it. What should or can he do?

You should report it to the police and ask them to refer it to the District Attorney's Office for prosecution.
You can also file a civil suite to recover those funds. Also, if she signed the promissory note in front of a notary public or in front of the lender as you, they could also possibly be liable for a variety of reasons via conspiracy.
