Legal Question in Family Law in North Carolina

i have a family member that is 23 has a IQ of a 7 year old. she also has a 9 month old, she is going to be moving into a group home but can't take the baby with her. i would like to no what i need to do to be able to keep the baby she is willing to sign papers so that the baby can stay with me
