If a mediation agreement is in place and a parent violates it by changing her location and her phone number without notifying the other parent and does not report to the court date what normally happens ?

It depends on the terms of the mediation agreement. Breach of a mediation agreement generally will result in possibly being forced to comply and generally not much more than that however, missing a court date could result in all sorts of bad things happening.
