Do you have to provided full names and adresses of people you have slept with while seperated? NC

Not unlless you are Court ordered. Even then you may be able to refuse to pony up the info by asserting your 5th ammendment right to not incriminate yourself - fornication/adultrey is a class 2 misdemeanor in hillbilly North Carolina.
