Legal Question in Medical Leave in North Carolina

Family Medical Leave Act

I've been working at a well known department store for four months. I developed a hernia and requested a medical leave. They told me to write a letter, stating my reason and start date. Aren't they suppose to give FMLA forms?

No, you would not be eligible until you have worked there at least a year. (FMLA leave also doesn't entitle you to pay during the time you are out.)
