You should consult with a real estate attorney in your area. There are (slightly exagerrating), a million ways to convey land and property. Each method has several pros and cons involved. You have to determine why you are giving the land to her (estate planning, gift, avoiding creditors), and from there consult with an attorney who can tell you what method you should employ. A title search on the property should also be considered unless you are 100% comfortable with the status of the title.
