Is there a pre-printed form that can be downloaded to give someone lifetime rights and control to a property.

What you seem to be talking about is giving them lifetime rights to land on a deed. This can be done by drafting a new deed. Generally, if the land is owned free and clear, you can convey it with lifetime rights and the remainder to someone else. However, there may be other issues relating to gift tax or Medicaid implications which you need to be aware of before the conveyance is made. You also may not even need a deed but a will or trust to accomplish your goals.
If you draft your own, there are forms, some of which are free and some of which are not. Since I don't know enough about your situation, I don't want to recommend anything as I don't know if its applicable or not.
