Can a husband and wife use the same living will form or would they need to do a separate one?

No there are no joint living will forms. Husband and wife have to do their own. Here is a link to the free form available at the North Carolina Secretary of State: http://www.nclifelinks.org.
The completed form can be filed at the Secretary of State's office for a fee of $10 for each of you ($10 for husband and $10 for wife for a total of $20). Directions are at the Secretary of State's website.
I find the official state form to be confusing and I have combined my living will/advanced directive with a health care power of attorney. It eliminates any problems of one document overriding the other and typically, especially in a case where people are married, the same person is in charge of both health care and end of life decisions.
