If you cannot complete this form, then you do not know what you are doing and need a probate attorney. Or to at least pay a probate attorney for a con sult.
This is a form for use in simple estates that a personal representative uses to collect/disburse assets without the need to go through a full blown probate. Assets collected are listed in part II. Example, if the deceased had a bank account at PNC Bank that had $300 on it, you get the $300 from the bank as you are the personal representative, then you list $300 received from the bank on the form. You would do that with all property received into the estate, like a car or anything else of value. You then put the total down for all assets received. You then indicate what you spent - so much for funeral, medical whatever. Whatever assets are left are then distributed to heirs/beneficiaries.
