i am wanting for the approver of my surgery for my back i hurt it at work . what do i do ? is their forms that i need or what. am i what on the insurance co. and i have a surgery date of july 6, 2009. what do i do

If your claim has been filed on form 18 and your injury is accepted as compensable on form 60 or 63, the doctor's office will normally contact the adjuster to get authorization. If not approved promptly, you may have to request a hearing. Get a board certified specialist in workers' compensation law to represent you ASAP.
