Paternity custody unmarried couple

My grandson wants to file for custody of his son, who lives with him. We do not know which forms to file with the court, the only forms I see at findlegalforms.com are custody with divorce. Are those forms OK to use or do we need a special form because he is not married to the mother?

The mother lives in AZ but is on drugs and cannot take care of the baby.

Divorce forms would not be appropriate if they have never been married. Instead, he would need to establsh "Paternity", and then would need to seek Orders regarding custody, access and child support.
