To form an Ohio LLC you need to file articles of organization with the Ohio Secretary of State and pay the required fee. The forms for the Articles can be found on the Secrtary of State home page.

You will also need an Operating Agreement which contains information on how the company will be run who will manage it, the powers the manager has, what happens if someone dies or leaves the company, etc. The terms of the Operating Agreement are usually very particular to the company you are going to form and are best drafted by an attorney.

You may also need to apply for a taxpayer identification number (IRS SS-4).

You may also need transfer documents to transfer assets into the LLC.
