If a student has a 504 plan and fails a class, can the grade be contested if the teachers did not follow the 504 plan?

Probably -- If your student is still in K-12, I would contact someone in special education at your school district. If in post-secondary, I would contact whatever department at the school that is responsible for accommodation services. I would try to work with these folks informally. It has been my experience that they will do their best to advocate on behalf of your student.
