Where can I find a legal form to file for custody?

There is no "standard form" available to file for a change in custody. You must file a motion with the appropriate court with jurisdiction, either juvenile or domestic relations, and support tat motion with an affidavit detailing the reasons under the law why custody should be changed. These documents must be filed with the court, and served on the opposing party within three days thereafter by certified mail. You can't just fill in the blanks, and hope to succeed.
