Name Change
When filing for a divorce or dissolution is the women automatically allowed to take back her maiden name or must she specifically request or file for the right to take back her maiden name?

Getting your maiden name back is not automatic. It is however, merely a one line entry to change your name back. If your want to do it later, you will have to go to probate court to change your name.
