Child name change
I would like to know how can I have my daughters last name changed legally, to my husbands. On her birth certificate father's name remains blank.
Place of birth is Patchouge, NewYork.

You need to file the motion in your local probate court, and serve notice on the father of the child, unless father has had no support or contact in over one year.
