Child Name Changing

Can you tell me where I would find online, the laws pertaining to a mother changing a childs name (when the child has the fathers name) back to moms maiden name without the fathers permission and/or knowledge.

Look in the probate code. You have to make some attempt to contact the natural father prior to changing the name. The forms are at probate court.
