convertion from a disolution to a divorce

I filed in Ohio a disolution of marriage and I was told by the court that what I need is a Divorce since my exhusband is in prison. Now I have to file a convertion from a disolution to a divorce. Where do I find the form. I have no means to hire an attorney. I did the disolution myself because I found all the forms online. Where online can I go to get this form. Please help me.
