Custodial prohibition

What specific things can keep a father from being able to see his child? What rights does a parent have? What, if anything, can be done to ensure visitation rights if the parents were never married?

Unwed father's have no (i.e. zero) rights until they establish paternity and then establish their visitation rights in a court.
